Job Title: Chaplain/Spiritual Care Coordinator

Job Purpose
To provide spiritual counseling to meet the assessed spiritual needs in accordance with the patient and family’s acceptance of this service, and in a manner consistent with the patient and family beliefs and desires.
Duties and Responsibilities
- Provide an assessment of each patient and family’s spiritual needs.
- Develop a spiritual Plan of Care based on identified needs.
- Make all reasonable efforts to facilitate visits by local clergy, pastoral counselors, or other individuals who can support the patient’s spiritual needs.
- Ensure patients and families know of the availability of spiritual care services.
- Provide direct spiritual support as needed/wanted to patients and families.
- As a member of the Interdisciplinary Group (IDG), provide updates to the patient’s spiritual Plan of Care.
- Provide bereavement follow-up services as needed.
- Within 24 hours, document in the patient’s chart any contact with patient or family.
- Conduct or make arrangements for funeral or memorial services as needed.
- Provide regular memorial services to nursing homes and assist with the annual Hospice of NCO memorial service.
Qualifications
- Graduate of an accredited seminary or school of theology preferred.
- Minimum of one professional or academic training in the field of ministry, chaplaincy, or spiritual support.
- Experience providing spiritual care and support.
- Ability to be open, flexible, and ecumenical, supporting the spiritual needs of individuals from any and all faith backgrounds.
- Possession of a valid driver’s license and auto insurance, with the ability to visit patient’s homes.
