Hospice Aide
Hospice of North Central Oklahoma is seeking a Certified Nursing Assistant to provide personal care and homemaker services maintaining a safe, healthy environment for the patients.
Duties and responsibilities
Hospice aides are assigned to a specific patient by a registered nurse that is a member of the interdisciplinary group. Written patient care instructions for a hospice aide must be prepared by a registered nurse who is responsible for the supervision of a hospice aide as specified under paragraph (h) of this section.

A Hospice Aide Provides Services that Are:
- Ordered by the interdisciplinary group.
- Included in the plan of care.
- Permitted to be performed under State law by such hospice aide.
- Consistent with the hospice aide training.
- The duties of a hospice aide include the following: o The provision of hands-on personal care.
- The performance of simple procedures as an extension of therapy or nursing services.
- Assistance in ambulation or exercises.
- Homemaker services as outlined in the patient Plan of Care
- Hospice aides must report changes in the patient’s medical, nursing, rehabilitative, and social needs to a registered nurse, as the changes relate to the plan of care and quality assessment and improvement activities. Hospice aides must also complete appropriate records in compliance with hospice’s policies and procedures.
Qualifications
- Licensed as a Certified Nursing Assistant by the State of Oklahoma.
- Certified as a Home Health Aide by the State of Oklahoma.
- Certified Hospice & Palliative Nurse Assistant preferred.
- Clean criminal background check and fingerprints on file with the State Health Department.
- Demonstrated effective communication, leadership and problem-solving skills.
- Demonstrated skills as exhibited by passing of agency exam with score of 75 percent or higher as well as demonstrating skills required on agency checklist.
- Possession of a current driver’s license for the State of Oklahoma, vehicle insurance and the ability to visit the homes of patients.
- Must be highly motivated, energetic, and work interdependently and collaboratively. Requires exceptional attention to detail and communication skills. Requires basic time management and organizational philosophy understand, along with policies and procedures.
Full-Time LPN Float Nurse
Hospice of North Central Oklahoma is seeking a Full-Time LPN Float Nurse. Candidate must be available Monday – Friday 8:00 a.m. to 4:30 p.m. As a member of the interdisciplinary team, the Hospice Float nurse works under the general direction the Director of Nursing and the attending physician in the provision of skilled nursing assessment, planning, and care in order to maximize the comfort and health of patients and families. Duties and Responsibilities
- The LPN staff nurse is responsible for identifying patient/family needs and for providing supportive care in accordance with the attending physician's orders, the plan of care as established by the plan of care and Hospice of North Central Oklahoma, Inc.’s policies and procedures.
- LPN Nurse will complete referrals, discharges, death, triage, patient care and follow up with phone calls of all required documentation within the scope of practice. Will provide patient care with focus on patient comfort and pain and symptom management.
- Provides documentation of care through timely charting in the electronic medical record and update care plans as necessary.
- Initiates communication with attending physicians, other hospice staff members, and other agencies as needed to coordinate optimal care and use of resources for the patient/family.
- Obtains data on physical, psychological, social, and spiritual factors that may influence patient/family health status and incorporate that data into the plan of care and reports to family services/chaplain when appropriate.
- Maintains up-to-date charting so that problems, plans, actions, and goals are accurately and clearly stated and changes are reflected as they occur. Documents care and services within a time frame consistent with agency policy.
- Seeks input from other interdisciplinary group members regarding the patient plan of care to obtain additional knowledge.
- Attends interdisciplinary group meetings and is involved in discussion of patient care for full staff discussion, consultation, and evaluation.
- Provides appropriate support to families and caregivers at the time of death and period of bereavement. Within scope of practice, the LPN nurse will assist with after death needs of patient family, including calling funeral home or other personnel and completion of documentation.
- Assesses, manages and administers patient’s medications following Policy and procedures of HNCO.
- Maintains a cooperative working relationship with other personnel involved In providing care to hospice patients.
- Will be included on rotating schedule for holidays and if applicable, weekend coverage for on-call PTO.
- Participates in Hospice of North Central Oklahoma, Inc.’s orientation and in-service training programs for professional staff and participates in agency and community programs as requested to promote professional growth and understanding of hospice care.
- Demonstrates familiarity with policies of the agency and rules and regulations of State and Federal bodies.
- Participates in quality improvement activities as requested by Director of Nursing.
Qualifications
- Possession of a current license to practice as a licensed practical nurse in the State of Oklahoma.
- Prefer two years of varied work experience as a professional nurse, with one of the two years in a hospice or home health setting.
- Demonstrated ability to assess and respond to the needs of patients and families in varied settings.
- Demonstrated skill in nursing practice, ability to cope with family emotional stress, and tolerance of individual lifestyles.
- Possession of a current driver's license for the State of Oklahoma and proof of vehicle insurance and ability to visit the homes of patients.
